Knowledge, Skills and Abilities:

·         Develop, review, monitor and evaluate Individual Support Plan (ISP) in accordance with mandated timeliness.

·         Compiles information from a variety of sources to make recommendations for eligibility determination.  Methods, procedures, and techniques used in the development of Individual Support Plans (ISP)

·         Federal and State statutes and agency policies and procedures relating to the care and assistance for individuals with developmental disabilities

·         Health Insurance Portability and Accountability Act (HIPAA)

·         Developing, evaluating, and maintaining member service plans

·         Oral and written communication

·         Interviewing, counseling, and interacting with members and family members

·         Interpersonal relations as applied to contacts with members and representatives of other governmental and community organizations and the public

·         Conducting needs assessments, Problem identification and resolution, Conflict resolution

·         The use of various software (Microsoft Word, Excel, etc.)

·         Principles and practices of case management, Principles of applied behavioral interventions, Community resources, Child development principles

·         Medical and behavioral health issues and terminology

·         Work independently and within diverse work teams

·         Elicit accurate data from a variety of sources

·         Absorb, interpret, and assimilate large quantities of written material

·         Organize and manage case load

·         Document case files and information within required/mandated timelines

 

Qualifications:

·         Knowledgeable in federal and state statues and agency policies and procedures

·         Possess a bachelor’s degree in Social Work or a bachelor’s degree with two years of case management experience serving persons with physical or developmental disabilities

·         Possess a valid level one fingerprint clearance card issued pursuant to Arizona Revised Statute 41-2758.07 in order to work with children and vulnerable adults

·         Candidates for this position shall be subject to a search of both the Child Protective Services Central Registry pursuant to A.R.S. 8-804 and the Adult Protective Services Registry pursuant to A.R.S. 46-459

·         Have a current, valid state issued driver’s license appropriate to the assignment. Employee is subject to driver’s license record checks and must maintain an acceptable driving record, valid motor vehicle insurance and registration, and must complete any required training (see Arizona Administrative Code R2-10-207.12)

·         Successfully complete the Electronic Employment Eligibility Verification Program (E-Verify), applicable to all newly hired State employees

·         Successfully pass background and reference checks. Employment is contingent upon completion of the above-mentioned process and the agency’s ability to reasonably accommodate any restrictions
