CASE MANAGER 2 - ELIGIBILITY

JOB SUMMARY:

The Division of Developmental Disabilities’ (DDD) Case Manager 2 responsible for serving members with developmental disabilities in the Eligibility Unit by supporting them and their families to lead the healthiest, most self-sufficient, productive lives within their capabilities.

JOB DUTIES: 

-          Conducts eligibility interviews; reviews eligibility referral/redetermination; tracks redetermination; compiles information regarding eligibility determination/redetermination from a variety of sources in order to make recommendations on DDD eligibility determinations

-          Receives and screens referrals and refers to community resources

-          Writes comprehensive progress notes, sets-up new member files

-          Reviews documentation, makes eligibility determinations, researches disability diagnoses

-          Correspondence with families on initial applications, denial of service notices, etc.  Attends training and meetings

 

KNOWLEDGE, SKILLS AND ABILITIES (KSAs):

Knowledge of:

-          Federal and State statutes and agency policies and procedures relating to the care and assistance for individuals with developmental disabilities

-          Medical and behavioral health issues and terminology

 Skill in:

-          Oral and written communication

-          Interviewing, counseling, and interacting with members and family members

-          Using various computer software (Microsoft Word, Excel, etc.)

 Ability to:

-          Work independently and within diverse work teams

-          Organize and manage eligibility case load

-          Drive a vehicle

SELECTIVE PREFERENCE(S):

-          The candidate must possess two (2) years of professional experience coordinating social service activities (e.g., ISPs, client referral for services, community services, etc.); OR one (1) year as a Human Service Specialist 1 in Arizona State Service; OR a Bachelor’s or Master’s degree from an accredited college or university.

 

PRE-EMPLOYMENT REQUIREMENTS: 

-          Have a current, valid state issued driver’s license appropriate to the assignment. Employee is subject to driver’s license record checks and must maintain an acceptable driving record, valid motor vehicle insurance and registration, and must complete any required training (see Arizona Administrative Code R2-10-207.12)

-          Successfully complete the Electronic Employment Eligibility Verification Program (E-Verify), applicable to all newly hired State employees

-          Successfully pass background and reference checks. Employment is contingent upon completion of the above-mentioned process and the agency’s ability to reasonably accommodate any restrictions

-          Employee shall have a valid level one fingerprint clearance card issued pursuant to Arizona Revised Statute 41-2758.07 in order to work with children and vulnerable adults

-          Candidates for this position shall be subject to a search of both the Child Protective Services Central Registry pursuant to A.R.S. 8-804 and the Adult Protective Services Registry pursuant to A.R.S. 46-459
