JOB SUMMARY:

This position serves members with developmental disabilities, e.g., Autism, Cerebral Palsy, Intellectual Disability (for example, Down Syndrome), Epilepsy, etc., by supporting them and their families to lead the healthiest, most self-sufficient, productive lives within their capabilities.

JOB DUTIES: 

-          Conducts on-site visits to members’ and/or families’ homes, schools and program sites to develop relationships and family dynamics; discusses service needs and supports; develops and writes the Individual Support Plan (ISP); documents individual progress and concerns; and completes appropriate referrals to community agencies

-          Researches community for available resources to meet needs identified in ISP and to inform member of resource options; facilitates meetings and program visits to assist with member’s and/or family’s choices; completes referrals for assessed/covered services

-          Develops, reviews, monitors and evaluates ISP in accordance with mandated timeliness; approves type and level of assessed services within approved guidelines; and release client funds

-          Collaborates with community agencies to provide assistance and support to members and families in crisis; attends meetings with members/families, service providers, and others as required; identifies and resolves issues/conflicts

-          Writes comprehensive progress notes and maintains case records

-          Compiles information from a variety of sources in order to make recommendations for eligibility determination and re-determinations of eligibility

KNOWLEDGE, SKILLS AND ABILITIES (KSAs):

Knowledge of:

-          Methods, procedures, and techniques used in the development of Individual Support Plans (ISP)

-          Health Insurance Portability and Accountability Act (HIPAA)

-          Medical and behavioral health issues and terminology

 Skill in:

-          Oral and written communication

-          Interpersonal relations as applied to contacts with members and representatives of other governmental and community organizations and the public

-          The use of various software (Microsoft Word, Excel, etc.)

Ability to:

-          Work independently and within diverse work teams

-          Absorb, interpret, and assimilate large quantities of written material

-          Maintain confidentiality

PRE-EMPLOYMENT REQUIREMENTS: 

-          The candidate must possess a Bachelor’s degree in Social Work; OR any Bachelor’s degree with 2 years of case management experience serving persons with physical or developmental disabilities.

-          Have a current, valid state issued driver’s license appropriate to the assignment

-          Employee is subject to driver’s license record checks and must maintain an acceptable driving record, valid motor vehicle insurance and registration, and must complete any required training (see Arizona Administrative Code R2-10-207.12) 

-          Successfully complete the Electronic Employment Eligibility Verification Program (E-Verify), applicable to all newly hired State employees

-          Successfully pass background and reference checks; employment is contingent upon completion of the above-mentioned process and the agency’s ability to reasonably accommodate any restrictions  

-          Employee shall have a valid level one fingerprint clearance card issued pursuant to Arizona Revised Statute 41-2758.07 in order to work with children and vulnerable adults 

-          Candidates for this position shall be subject to a search of both the Child Protective Services Central Registry pursuant to A.R.S. 8-804 and the Adult Protective Services Registry pursuant to A.R.S. 46-459
