The Complex Care Registered Nurse (RN) is responsible for clinical oversight of care coordination of members with both physical and behavioral health needs, the issues of cost, serious medical problems and high utilization of health care services.  The Complex Care RN is responsible for providing comprehensive care coordination and monitoring (treatment planning case management) to individuals having Intellectual Developmental Disability, many with co-existing physical and behavioral needs. This position requires a dynamic, proactive approach to assessment, monitoring and coordination of care, appropriate placement, discharge planning, to ensure quality supports and consistent adherence to waiver requirements. This is a mobile position with work done in a variety of locations and spends a considerable amount of time in the field and completes required documentation/paperwork.


-          Provides technical assistance/consultative clinical support to care coordination within the Division and within other systems of care by attending Child and Family Team/Adult Recovery Team meetings (CFT/ART), and other state level meetings

-          Coach support coordinators on the job and enhance the Division of Developmental Disabilities (DDD) integrated care delivery

-          Function as a liaison to internal departments to timely address the needs of each person including Support Coordination, Network, Behavioral Health Units, and Nursing

-          Assists in addressing behavioral health portions of the Arizona Health Care Cost Containment System (AHCCCS) and Acute Care Contracts related to oversight and monitoring of integration of physical and behavioral healthcare services to DDD members

-          Behavioral health performance improvement programs, initiative and measure including completion of contract reports and work plan goals

-          Works directly with network resources to obtain proper placement settings for members with complex care and ensures appropriate support servics are in place

-          Monitors/tracks inpatient psychiatric services, residential treatment services, including out-of-state placements, court ordered treatment, incarcerated members, as identified by the Division's Behavioral Health Unit or by contract

-          Assist in development/tracking of behavioral health goals on District Network Plan


Knowledge of:

-          Nursing theories and practices related to individuals with developmental disabilities and behavioral health conditions

-          Problem solving, negotiation and conflict resolution skills are essential to balance the needs of both internal and external customers

-          Proficiency in Microsoft Office products (such as Word, Excel, Outlook, etc.)

Skilled in:

-          Actively participating in the care planning process, updating care coordination meetings and calls

-          Consistently being able to work calmly and maintain effectiveness in situations of high stress

-          Understanding appropriate placement settings based on the assessed needs of the member

-          Exceptional interpersonal skills, highly effective communication ability, and the propensity to make prompt independent decisions based upon relevant facts and established processes

Ability to:

-          Interact with the public and other employees in a positive manner and promote a cooperative working environment

-          Handle sensitive information, maintain confidentiality

-          Communicate effectively orally and in writing

-          Work independently with minimal supervision

-          Manage work load and complete tasks in a timely manner

-          Partner with stakeholders, Department of Child Safety (DCS), AHCCCS, Comprehensive Medical and Dental Plans (CMDP), Arizona Department of Corrections (ADOC), etc. for cross system collaboration

-          Be detail oriented, to independently organize multiple tasks and priorities and to effectively manage and assigned caseload under pressure of deadlines


-          A Bachelor's of Science in Nursing (BSN)

-          Two (2) or more years of professional experience with developmental disabilities or seriously mentally ill


-          Possession of a current license or temporary permit to practice as a registered nurse in the State of Arizona, OR within six (6) months from date of hire, must possess a current license to practices as a registered nurse in the State of Arizona

-          Have a current, valid state issued driver’s license appropriate to the assignment. Employee is subject to driver’s license record checks and must maintain an acceptable driving record, valid motor vehicle insurance and registration, and must complete any required training (see Arizona Administrative Code R2-10-207.12)

-          Successfully complete the Electronic Employment Eligibility Verification Program (E-Verify), applicable to all newly hired State employees

-          Successfully pass background and reference checks. Employment is contingent upon completion of the above-mentioned process and the agency’s ability to reasonably accommodate any restrictions

-          Employee shall have a valid level one fingerprint clearance card issued pursuant to Arizona Revised Statute 41-2758.07 in order to work with children and vulnerable adults

-          Candidates for this position shall be subject to a search of both the Child Protective Services Central Registry pursuant to A.R.S. 8-804 and the Adult Protective Services Registry pursuant to A.R.S. 46-459 

-          Employment is contingent upon the ability to lift 50 pounds or more and the agency’s ability to reasonably accommodate any restrictions.
