Job Summary: The Department of Economic Security Division of Developmental Disabilities is seeking experienced and highly motivated individuals to join our team serving members with developmental disabilities (i.e. autism, cerebral palsy, intellectual disability or epilepsy) ranging from mild to profound. The position assists members and their families to lead the healthiest, most self-sufficient, productive lives within their capabilities.
Job Duties: · Drive a vehicle to conduct onsite visits to members’ homes, schools and/or program sites to develop relationships and to discuss service needs and supports · Develop, implement, monitor, and evaluate Individual Support Plans (ISPs) in accordance with mandated timelines · Write comprehensive progress notes and maintain case records; prepare a variety of reports and complete special projects · Click HERE to see a day in the life of a Division of Developmental Disabilities Case Manager
Knowledge, Skills, Abilities/Qualifications/Requirements: · Knowledge in 1) HIPAA laws, 2) Medical and behavioral issues and terminology · Skilled in 1) Oral and written communication, 2) Conducting needs assessments, 3) Problem identification and resolution · Ability to 1) Work independently and within diverse work teams, 2) Document case files and information · Candidate must be an Arizona licensed Registered Nurse in good standing OR possess a Bachelor Degree in Social Work (SW), OR Psychology, Special Education or Counseling AND have at least one (1) year experience providing case management services to persons who are elderly and/or persons with physical or developmental disabilities and/or members determined to have a Serious Mental Illness (SMI) OR have a minimum of two (2) years’ experience providing case management services to persons who are elderly and/or persons with physical or developmental disabilities and/or members determined to have a Serious Mental Illness (SMI) · Arizona Level 1 Clearance Card is required · Requires a valid State-issued driver’s license · Successfully pass background and reference check; employment is contingent upon completion of the above-mentioned process and the Agency’s ability to reasonably accommodate any restrictions
