Job Summary: Functions as staff support in the Division of Developmental Disabilities (DDD) Eligibility Units assisting with casework directed by the Unit Supervisors, Human Service Specialists, and administrative and secretarial staff. This position is responsible for completing clear and detailed documentation; maintaining documentation and files; completing eligibility and re-redetermination packets; scheduling activities for the work unit; providing accurate and professional responses to inquiries from the public and agency staff and completing brief reports as necessary.

 

Job Duties:

·     Processes Eligibility packets. Document received packets/cases into unit log Routinely checks database for a record of a referral/ application. Reviews application packets for completeness. Forwards completed packets to the assigned Eligibility Specialist and creates proxies for flowboard.

·     Conduct daily follow up with providers to obtain additional information that is needed for processing of Eligibility packets.

·     Close case files to assist with determinations within time frames outlined in policy and procedure.

·     Serves as the in-day person in a unit and/or office. Answers office telephones and phone calls for Eligibility Specialists who are out of the office. Takes accurate messages and/or routes to appropriate staff. Responds to routine questions for information from families and providers.

·     Maintains office supplies, copy machines, postage meter, etc. Opens, reviews, sorts, and distributes office mail to appropriate Unit staff, ensuring that critical documents (e.g., applications, medical records) are prioritized for a timely response by the Eligibility Specialist/Division. Copies documents as requested.

·     Collects necessary information for and prepares eligibility and member re-determination packets. Tracks eligibility decisions. Inputs member information into member information database.

·     Prepares a wide variety of unit documents (e.g., correspondence, memos and forms) for signature of designated authority for work unit and/or office. Responds to requests from various sources to update the prepared information.

·     Other duties as assigned, as related to the position   

 

Knowledge, Skills, Abilities/Qualifications/Requirements:

·         Knowledge in Basic human needs, human growth, personality, and behavior

·         Skill in Applying federal laws, state statutes, and agency policies and procedures

·         Recognizing basic human rights and the care and assistance needs of individuals with developmental     disabilities

·         Oral and written communication

·         Organizational skills 

·         Initiating, developing, and maintaining member files 

·         Interpersonal relations as applied to contacts with applicants, members and representatives of other     governmental and community organizations and the public

·         Basic clerical skills including proper telephone etiquette, operating office equipment (e.g., copier, fax)

·         Microsoft Word, Outlook, Excel, and other computer applications

·         Abilitiy in Adapt and be sensitive to a variety of cultures

·         Maintain and display professional demeanor

·         Establish family and professional partnerships

·         Work effectively with a wide range of personalities

·         Assess/review documents to determine importance to meeting member/family needs

·         Type a minimum of 40 wpm

 

Licenses / Certifications:

·        Candidates for this position shall be subject to a search of the Child Protective Services Central Registry pursuant to     A.R.S. 8-804.

·        Employee shall have a valid Level One fingerprint clearance card issued pursuant to Arizona Revised Statute    41-2758.07 in order to work with children and vulnerable adults.

·        Travel may be required for State business. Employees who drive on state business must complete any required    driver training (see Arizona Administrative Code R2-10-207.12.) AND have an acceptable driving record in accordance    with DES Fleet Management-Safety Program policy and procedures (DES 1-07-26 & DES 1-07-26-01). Employees may    be required to use their own transportation as well as maintaining valid motor vehicle insurance and current Arizona    vehicle registration; however, mileage will be reimbursed
