Job Summary:  The Department of Economic Security Division of Developmental Disabilities is seeking an experienced and highly motivated individual to join our team as a Case Manager 2. This position is responsible for serving members with developmental disabilities, e.g., Autism, Cerebral Palsy, Intellectual Disability (for example, Down Syndrome), Epilepsy, etc., by supporting them and their families to lead the healthiest, most self-sufficient, productive lives within their capabilities. Job Duties:

·        Drive a vehicle to conduct on-site visits to members’/families’ homes, schools and program sites to develop relationships and family dynamics; discuss service needs and supports; develop and write the Individual Support Plan (ISP); document individual progress and concerns; and complete appropriate referrals to community agencies

·        Research community for available resources to meet needs identified in ISP and to inform member of resource options; facilitate meetings and program visits to assist with member/family choices; complete referrals for assessed/covered services

·        Develop, review, monitor and evaluate ISP in accordance with mandated timeliness; approve type and level of assessed services within approved guidelines; and release client funds

·        Collaborate with community agencies to provide assistance and support to members and families in crisis; attend meetings with members/families, service providers, and others as required; identify and resolve issues/conflicts

·        Write comprehensive progress notes and maintain case records; prepare a variety of reports and complete special projects, as required

·        Compile information from a variety of sources in order to make recommendations for eligibility determination and re-determinations of eligibility; confer with supervisor, attend staff meetings, training and other meetings, as required

 

Knowledge, Skills, Abilities/Qualifications/Requirements:

·        Knowledge in 1) Methods, procedures, and techniques used in the development of Individual Support Plans (ISP), 2) Federal and State statutes and agency policies and procedures relating to the care and assistance for individuals with developmental disabilities, 3) Health Insurance Portability and Accountability Act (HIPAA), 4) Principles and practices of case management, 5) Community resources,  6) Medical and behavioral health issues and terminology, 7) Principles of applied behavioral interventions, 8) Child development principles

·        Skilled in 1) Oral and written communication, 2) Developing, evaluating, and maintaining member service plans, 3) Interviewing, counseling, and interacting with members and family members, 4) Conducting needs assessments, 5) Problem identification and resolution, 6) Interpersonal relations as applied to contacts with members and representatives of other governmental and community organizations and the public, 7) Conflict resolution, 8) The use of various software (Microsoft Word, Excel, etc.)

·        Ability to 1) Work independently and within diverse work teams, 2) Elicit accurate data from a variety of sources, 3) Absorb, interpret, and assimilate large quantities of written material, 4) Organize and manage case load, 5) Document case files and information within required/mandated timelines, 6) Maintain confidentiality

·        As required by AHCCCS Contract No. YH6-0014, a Case Manager must have a degree in Social Work; OR be a Licensed Nurse; OR have a bachelor's degree in Psychology, or Special Education, or Counseling AND one (1) year Case Manager experience serving persons who are elderly and/or persons with physical or developmental disabilities and/or members determined to have a Serious Mental Illness (SMI); OR have two (2) years Case Manager experience serving persons who are elderly and/or persons with physical or developmental disabilities and/or members determined to have a Serious Mental Illness (SMI)

·        For Case Managers who will serve persons who are elderly and/or persons with physical or developmental disabilities and have been determined to have an SMI the requirement is as follows:

1.      One (1) year of case management experience serving elderly and/or persons with physical or developmental disabilities, AND Two (2) years of case management experience serving members determined to have an SMI

·        Arizona Level 1 Clearance Card is required

·        In-state travel is required

·        Candidates for this position shall be subject to a search of both the Child Protective Services Central Registry pursuant to A.R.S. 8-804 and the Adult Protective Services Registry pursuant to A.R.S. 46-459

·        Successfully complete the Electronic Employment Eligibility Verification Program (E-Verify), applicable to all newly hired state employees

·        Successfully pass background and reference check; employment is contingent upon completion of the above-mentioned process and the Agency’s ability to reasonably accommodate any restrictions
