The Department of Economic Security, Division of Developmental Disabilities (DDD) is seeking an experienced and highly motivated individual to join our team as a Case Manager Lead – Early Intervention.

 This position is responsible for:

·        Driving a vehicle to conduct on-site home visits to meet with member/families to develop relationships and become familiar with family dynamics/needs, and making appropriate referrals to community agencies; facilitating multi-disciplinary Individual Family Support Plan (IFSP) meetings for member/children (minimum of every 6 months on every child) to determine and identify services for member; working with Department of Child Safety (DCS) to meet child's need while in the system

·        Researching community for resources to meet needs identified in plan, informs member/family of options; facilitating meetings and program visits to enable member/family to choose appropriate providers; assisting family with enrollment into DDD Early Intervention Program and securing identified/needed services for all children/member under DDD and Arizona Long Term Care System (ALTCS); writing referrals for chosen providers to ensure funding is secured for services; conducting on-site visits with schools and service providers

·        For Members: reviewing, monitoring and evaluating IFSP in accordance with mandated timeliness. For Child: gathering all necessary information and documentation to determine eligibility for DDD Early Intervention; determining eligibility for children birth to 3 years of age; provideing “interim service coordination” for children and families who may not be eligible for DDD; completing re-determinations when appropriate.

·        Making on-site visits to member/family homes, schools and program sites to discuss member’s progress, needs, IFSP, etc.  For child: assessing each child's progress on a regular basis

·        Providing assistance and support to members/families in crisis; attending meetings with members/families, service providers and others as required; identifying and resolving problems

·        Compiling information from a variety of sources to make recommendations for eligibility determinations and re-determinations of eligibility

·        Writing comprehensive progress notes and maintains case records; preparing a variety of reports and completing special projects; developing and maintaining files for each child; attending staffing meetings, training and other meetings (including statewide travel); acting as Supervisor in Supervisor’s absence


Knowledge, Skills, Abilities:

·     Knowledge of methods, procedures and techniques used in the development of Individual Family Support Plans (IFSP)

·   Knowledge of Federal and State statutes and agency policies and procedures relating to the care and assistance for individuals with developmental disabilities

·        Knowledge of Health Insurance Portability and Accountability Act (HIPAA)

·        Knowledge of principles and practices of case management

·        Knowledge of community resources, medical and behavioral health issues and terminology

·        Knowledge of principles of applied behavioral interventions

·        Knowledge of child development principles

·        Knowledge of social, emotional, health, and/or educational needs of children

·        Skills in oral and written communication

·        Skills in initiating, developing and maintaining member service plans

·        Skills in interviewing, counseling and interacting with members and family members

·        Skills in coordinating and facilitating meetings

·        Skills in conducting needs assessments

·        Skills in problem identification and resolution

·  Skills in interpersonal relations as applied to contacts with members and representatives of other governmental and community organizations and the public

·        Skills in conflict resolution

·        Ability to work independently and within diverse work teams

·        Ability to maintain confidentiality

·        Ability to organize and manage a case load

·        Ability to document case files and information within required/mandated timelines

·        Ability to lead a team of Support Coordinators in the absence of the supervisor


Licenses and Certifications:


·        Candidate must have a minimum of a Bachelor’s degree in Early Childhood, Early Child Special Education, Family Studies, OR a Bachelor’s degree in a closely related field (e.g., Psychology, Social Work, Elementary Education, or Sociology) with review of course and approval by ADES/AzEIP; OR if hired in the AzEIP system as a Service Coordinator before July 1, 2001, documentation of continued satisfactory work in  the AzEIP system in a Service Coordinator position from that date of hire

·        Successfully complete the Electronic Employment Eligibility Verification Program (E-Verify), applicable to all newly hired State employees

·        Arizona Level 1 Clearance Card is required

·        Requires a valid State-issued driver’s license

·        Successfully pass background and reference check; employment is contingent upon completion of the above-mentioned process and the Agency’s ability to reasonably accommodate any restrictions
