The Department of Economic Security, Division of Developmental Disabilities (DDD) is seeking an experienced and highly motivated individual to join our team as a Performance Improvement Nurse.
This position is responsible for:
·
Gathering and organizing data associated with current project and processes; assisting in overseeing and/or conducting on-site visits to ensure an immediate safe environment for members and their physical and/or behavioral health needs are being met; assisting with the Division Health Plans for compliance to best practice and ensuring they are actively addressing any issues that arise
·
Assisting in the clinical triage of all incidents that flow into the Quality Management (QM) unit as well as their assignments to field staff for investigation; assisting with the Quality of Care (QOC) concern clinical investigations of Registered Nurse (RN) central office investigators through their second level review process; assisting in the teams' successful and timely completion of all QOC concerns and serious reportable events

Assisting manager with the development of comprehensive and collaborative project plans to reach across units (support coordination, complex care team, etc.), agencies, and provider networks to provide expertise as to the clinical fidelity of the quality management unit and to improve care delivery through documentation and education of not only DDD staff, but also of provider and sister agencies

Process Arizona Health Care Cost Containment System (AHCCCS) quality of care concerns and directly completing care concern case responses to AHCCCS as necessary; assisting with developing and implementing provider and community resources/outreach

Assisting in developing and documenting the policies, standard works, monitoring and metrics necessary for these processes including processes for elevating concerns to manager for review as well as through the appropriate committees of which this position will be an active participant and assisting with the prepping of all committee documentation to adhere to confidentiality standards

Providing assistance with mentoring and coaching of staff to ensure compliance with regulations and policies of DDD and departmental processes and guidelines to facilitate process improvements and other reports; assisting to ensure all fieldwork completed by RN investigators adheres to best practice, consistency in investigations and assists with the collection of all metrics, standard work, documentation and processes

Developing PowerPoints, newsletter articles, and other general information resources; providing on-site presentations to contracted providers (management and staff), support coordination, and community stakeholders
Knowledge, Skills, Abilities:

Licenses and Certifications:
·
Candidate must have a possession of a current license or temporary permit to practice as a Registered Nurse in the State of Arizona; within six (6) months from date of hire, must possess of a current license to practice as a registered nurse in the State of Arizona

Successfully complete the Electronic Employment Eligibility Verification Program (E-Verify), applicable to all newly hired State employees
·
Arizona Level 1 Clearance Card is required
·
Requires a valid State-issued driver’s license
·
Successfully pass background and reference check; employment is contingent upon completion of the above-mentioned process and the Agency’s ability to reasonably accommodate any restrictions
